I will participate in the En Coeur Foundation's 6H En Coeur. I agree to abide by the guidelines that will be in effect during the event and I release En Coeur, Quebec Foundation for Children with Heart Disease from any responsibility for falls or injuries that may be incurred during this challenge.
I acknowledge that I am personally responsible for my health and safety, as well as for the personal items I will bring. I agree to receive any medical attention that may be recommended to me in the event of illness or injury sustained during the challenge, and I agree to pay all medical expenses, if any.
I represent and warrant that I will be at least 18 years of age at the time of the challenge. If I am not 18 years of age, I understand that I must be accompanied by a registered adult participant during this challenge.
I grant permission, free of charge, to En Coeur, Quebec Foundation for Children with Heart Disease and its representatives to use my name, photo and voice free of charge in publicity, promotion or other reporting of the Challenge, or in the marketing or promotion of similar or future events.


EN COEUR, QUEBEC FOUNDATION FOR CHILDREN WITH HEART DISEASE RESERVES THE RIGHT TO MODIFY THE ORGANIZATION OF THE CHALLENGE OR TO CANCEL IT IN CASE OF FORCE MAJEURE FOR ANY OTHER REASON IT DEEMS APPROPRIATE. IN CASE OF CANCELLATION, THE REGISTRATION FEES WILL NOT BE REFUNDED.
