Who are we?
At Verdun Hospital, Notre Dame Hospital, in 14 CHSLDs and 8 CLSCs,the Fondation Santé Urbaine’s mission is to improve the mental and physical health of those living in the Centre-Sud of Montréal throughout their trajectory of care, from birth to end of life, through small acts of kindness, innovative projects, and the purchase of medical equipment.
The Fondation serves the over 300,000 residents of Plateau-Mont-Royal, Ville-Marie, Sud-Ouest and Verdun. It supoprts Notre‑Dame and Verdun hospitals, 14 CHSLDs and 8 CLSCs in these urban neighbourhoods and collaborates with over 18,000 employees of the Centre-Sud-de-l’Île-de-Montréal CIUSSS, including social workers, psychologists, doctors, nurses, occupational therapists and other specialists in the health network.
Vulnerable populations in the Centre-Sud of Montréal can count on the Fondation to be there at every stage of their life, for all of their needs, big or small. Here is just a sample of what we provide for our clients: massages for chemotherapy patients, grocery cards and medication payments for patients in financial distress, sustainable and innovative programs like our oncology patient transportation service, hospital-at-home care for mental health patients, a dental clinic for street youth and, last but not least, medical equipment like respirators and ultrasound machines.
