Declaration of Waiver and Release of Liability

In consideration of the authorization granted to me to participate in the PowerDog 2025 competition, hereinafter referred to as the “EVENT”, for the benefit of the Mira Foundation Inc. to benefit the Mira Foundation Inc. on June 7, 2025, I agree to abide by the rules and regulations of the “EVENT” and all applicable municipal and provincial by-laws.

I am aware of the dangers and I fully assume all risks associated with my participation in the “EVENT” including, without limitation, falls, injuries, contact with other participants, objects and vehicles, effects of weather and I affirm that my participation in the “EVENT” is voluntary.

In consideration of being permitted to participate in the “EVENT”, I hereby waive, on behalf of myself and any person authorized to act on my behalf, any and all claims for injuries and damages that may arise during the “EVENT” or because of my participation in the “EVENT” against the Mira Foundation Inc, PowerDog 2025, and any other organization, employees or volunteers involved in the “EVENT”.

I understand and agree that this waiver and release of liability is binding upon my heirs, guardians and legal representatives.

I certify that I have the physical, emotional and mental capacity to complete all elements of the “EVENT”. If I am aware of (or am being treated for) any physical infirmity, ailment or disease, my physician will have been notified and will have approved my participation in the “EVENT”. I acknowledge that I am personally and solely responsible for my health and safety and for the personal items I will bring with me, that my equipment, clothing and all accessories necessary or useful to enable me to participate are in good condition and are appropriate for these purposes. I consent to receive such medical attention as may be recommended in the event of illness or injury sustained by me during the Event and agree to assume all medical expenses.

I grant permission to the Mira Foundation Inc. and PowerDog 2025, without compensation, to use my name, photograph, voice or likeness in any report, promotional advertisement or other account of this event or for the marketing or promotion of similar or future events, and I hereby waive my right to image, anonymity and any confidentiality rights I may have in this regard. I acknowledge and consent to receive communications related to my participation in the “EVENT”.

I confirm that I am eighteen (18) years of age or older. In the event that the participant is less than eighteen (18) years of age, I, as his/her parent or legal guardian, confirm and declare that the minor meets the age criteria for participation in the Event as indicated by the Organizers and hereby agree to the foregoing.

I have carefully read this waiver and release and fully understand its contents.

In case of discrepancy or contradiction between this English version and the French version of this document, the French version prevails.
